Critical Time Intervention (CTI)

Helio Health’s Critical Time Intervention (CTI) is a short-term, phase-based, case management model that helps people during major life changes. Helio Health’s CTI program focuses primarily on those transitioning from a hospital setting. CTI connects you with the support you need to stay on track during your recovery journey.

CTI works by providing intensive support during the “critical time” or transitional period when individuals are at a heightened risk of instability. The team will work to strengthen the individual’s long-term goals of engaging with treatment and community supports while helping individuals build the confidence to navigate complex systems independently. CTI is a person-centered approach to collaboration, care, and empowerment. Collaboration is maintained with community services and supports.

CTI is for Adults 18+ being discharged from an inpatient psychiatric hospital, Emergency Room, or CPEP setting. Considerations can be made for individuals who are in crisis, precariously housed or homeless, or transitioning from incarceration/prison.

Supports currently provided in Oneida County. Individuals must be referred from inpatient psychiatric hospital, Emergency Room, or CPEP setting.
