Home and Community Based Services
Our Home and Community-Based Services (HCBS) for behavioral health and substance use disorders are provided in the community or in the home of the individual receiving the services. New York State designed these integrated Medicaid Managed Care benefits to help individuals move forward in their recovery.
How it Works
Each individual receives services that are recovery-oriented and evidenced-based. It is our intent to reach individuals where they are and help them to build lifelong skills. We assist individuals by exploring their interests, connecting them to services in the community, overcoming barriers, and by celebrating their strengths. An individual is eligible for Home and Community-Based Services if they are part of a Health Home, and if their Health Home has identified them as eligible for HCBS.
Habilitation/Residential Support Services
Helio Health Habilitation Services help individuals with substance use or mental health disorders maintain or achieve a residence in the community.
Services we offer include:
- Using community resources
- Developing and understanding budgets
- Facilitating familial communication
- Assistance with social support skills including strategies for maintaining recovery
- Strategies to manage trauma
- Any other aspect of rehabilitation that an individual wants to explore
Educational Support
Our Educational Support Services assist individuals with behavioral health challenges or a substance use disorder begin or return to school, learn a trade, or obtain training in order to reenter the workforce.
Services we offer include:
- Support in educational settings
- Assistance with obtaining financial aid or other resources
- Providing skill development in the areas of study skills, note taking, stress management, social skills and other areas as needed
- Assistance with the school setting and disability services
- Working on organizational skills and planning as they relate to education
- Investigating educational paths and engaging how to move forward
- Ongoing support in education setting
- Any other aspect of rehabilitation an individual wants to pursue
Prevocational Services
Our Prevocational Services are designed for those in recovery who are looking to explore new or different work experiences. These are time-limited services that help prepare individuals for volunteering or employment.
Services we offer include:
- Supporting work compliance, how to problem solve, and task completion
- Assistance in identifying obstacles to employment, and employment paperwork
- Working on skills necessary to keep a job, such as socialization, communication and career development
- Any other aspect of rehabilitation an individual wants to pursue
Transitional Employment
Our Transitional Employment services help individuals who have a behavioral health or substance use disorder obtain the skills they need to achieve competitive employment. We also focus on helping participants improve their non-job-related proficiencies to improve their chances of getting hired.
Services we offer include:
- On-the-job training
- Support to gain skills to obtain and maintain employment
- Assistance with finding transportation to and from work
- Discussing daily living skills, appropriate dress and proper conduct
- Providing support at the place of employment
- Any other aspect of rehabilitation an individual wants to pursue
Intensive Supportive Employment
Our Intensive Supportive Employment offers support to those recovering from mental health or substance use disorders so they can obtain and maintain gainful employment. Our program uses the evidenced-based Individual Placement Support model for training, which is supports-based and person-centered.
Services we offer include:
- Support surrounding obtaining hard and soft skills to retain employment
- Assistance establishing or maintaining self-employment
- Providing job placement, job coaching and other workforce support services
Ongoing Supportive Employment
Ongoing Supportive Employment is a service that is performed after an individual obtains competitive employment. This service is available for as long as an individual feels they need it.
Services we offer include:
- Help with obtaining reasonable accommodations
- Assistance building and maintaining skills in the workplace
- Assistance with positive work relationships and interactions with co-workers
- Assistance managing symptoms at the worksite
