Community Oriented Recovery and Empowerment Services
Our Community Oriented Recovery and Empowerment Services (CORE) for behavioral health and substance use disorders are provided in the community or in the home of the individual receiving the services. New York State designed these integrated Medicaid Managed Care benefits to help individuals move forward in their recovery.
How It Works
Each individual receives services that are recovery-oriented and evidenced-based. It is our intent to reach individuals where they are and help them to build lifelong skills. We assist individuals by exploring their interests, connecting them to services in the community, overcoming barriers, and by celebrating their strengths. An individual is eligible for Community Oriented Recovery and Empowerment Services if they are HARP enrolled or HARP eligible through their Managed Care Organization. There is a “no wrong door” referral pathway with CORE services, which means HARP members may learn about CORE Services through any number of sources, including through the Managed Care Organization (MCO), Health Home Care Manager (HHCM), inpatient and outpatient clinicians, primary care practitioners, family and friends, or provider outreach.
Empowerment Services — Peer Supports
Helio Health Empowerment Services — Peer Supports gives peers the opportunity to assist those struggling with their mental health or substance use disorder to move forward in recovery.
Services we offer include:
- Assistance obtaining benefits, food and shelter
- Working together to create a recovery lifestyle
- Connecting peers to various supports in the community
- Bridging from where an individual is now to where they want to be
- Providing crisis support
- Any other aspect of rehabilitation an individual wants to pursue
Psychosocial Rehabilitation
Helio Health Psychosocial Rehabilitation Services are intended to help the individual who is experiencing barriers with their behavioral health or substance use disorder. We work with people who are experiencing difficulty with interpersonal or social skills and how they interact with their environment.
Services we offer include:
- Establishing support networks
- Social skills training
- Interpersonal communication
- Recovery oriented activities
- Managing mental health symptoms
- Identifying triggers and finding positive outlets
- Developing new leisure activities
- Health and personal care
- Any other rehabilitation an individual wants to pursue
Family Support and Training
Helio Health Family Support and Training Services engage families in the recovery process. An individual must request this service. It is a person-centered, trauma-informed approach.
Services we offer include:
- Family support groups
- Assisting the family in understanding the mental health or substance use disorder of the individual
- Training on self-advocacy
- Education surrounding various workshops that are available
- Intervention strategies
- Any other aspect of rehabilitation that the individual wants to explore
Community Psychiatric Support and Treatment (CPST)
Community Psychiatric Support and Treatment includes goal-directed supports and solution-focused interventions with the intent to achieve person-centered goals and objectives provided in the community. This is a multi-component service that consists of therapeutic interventions such as clinical counseling and therapy, which assist the individual in achieving stability and functional improvement.
